I am unclear about what you
I am unclear about what you mean by "Product Release Form". My two guesses are
- a Sell Sheet
- a Press Release

Pretty sure the latter
The press release i think is what you're after, right?
In which case, Wombat is killin' it with those links.
You can also look at a nice service on fiverr to distrubute said press release, or to even write it for you (although you need to supply so much information you might as well write it yourself unless u no right gud).
