I made a template for plain cardstock "setup" boxes that I've been using, and figured I'd share it here:

it's designed to be printed on ordinary letter sized home-printer cardstock (though you can probably print it onto any size paper, the box will just be smaller), and cut out with scissors, and doesn't require tape or glue (though I reckon you can tape or glue it if you're so inclined).
Print out one for the top, one for the bottom, cut and fold them and you've got a box top and bottom. It will hold a couple of normal sized decks of playing cards, plus a few game pieces.
